Statement from the Client information bank register (the CIBR)

Would you like to know what information about your person is kept in the CIBR?

All you have to do is to fill in the Request for the CIBR’s Statement and deliver it to our address as follows:

  • personally – when you come in with your Request for the Statement personally, do not forget to take your valid proof of identity (preferably identity card).
  • By post – when you send the Request for the Statement by post, your signature on this Request has to be certified. The Statement is sent back into your hands on delivery and only to an address in the Czech Republic.
  • Via e-mail – there is also possibility to apply for the Statement via e-mail to address klient@cbcb.cz. Client center of the CIBR accepts an electronic filling for the Statement signed by your certified electronic signature based on a qualified certificate issued by the accredited provider of certificate services. Actual information about the assigned accreditation can be found here: http://www.mvcr.cz/mvcren/

We do not accept Requests for statements of information from the CIBR sent via data box at this moment. If you have an electronic signature for your data box, which fulfills the requirements mentioned above, you can send the Request via e-mail signed with this signature.

Request for correction of inaccurate or incorrect personal data in Client information bank register

Have you found out that the information kept about you in the CIBR is incorrect, inaccurate or incomplete?

Please turn to us with filled in request for correction of inaccurate or incorrect information and deliver it as follows:

  • personally (do not forget your valid proof of identity)
  • by post (the request sent by post has to be certified)
  • via e-mail to address klient@cbcb.cz (the request has to be signed with your qualified certificate, we also recommend to encrypt the message using the public certificate of the Client center. The public certificate of the Client center can be found here).

After you notify us about processing of incorrect or inaccurate information in the CIBR we will try to correct the entry in cooperation with the bank, as soon as possible. In case it will not be possible to correct the information, this incorrect or inaccurate information will be deleted without unnecessary delay. If we do not send you the information about the result of the investigation, eventually about the ongoing or finished correction of the mistake within 30 days since we receive your request for correction, you can consider your request to be solved to your satisfaction.

 

Who can request a service at Client center

Natural person, full aged and eligible to legal act. If any of these conditions is not met, the request can not be accepted. We require a valid identity card or passport as a proof of identity. Foreign natural person has to prove himself/herself with a valid passport or with a leave to permanent residence permit in the Czech Republic.

Natural person can be substituted in requesting the services and all relevant activities of client center by other person who submitted a legally verified letter of attorney (template here). The same proof of identity is requested from a substitute as it would be from the natural person the substitute acts for. If the applicant is a legal representative of the client, official document that proves this is required.
